Reliability of central venous pressure as an indicator of left atrial pressure. A study in patients with mitral valve disease.

چکیده
Central venous pressure (CVP) measurements have been used extensively in the management of critically ill patients. The value of CVP determinations was studied by determining the relationship of left atrial and right atrial pressares in 200 patients undergoing transsepta1 heart catheterization. All patients had mitral valve disease. The correlation coefficient of left atrial and right atrial pressure was 0.48. Low right atrial pressure (below 3 mm Hg) wss seen &I 18 of 91 patients with left atrial pressure 20 mm H g or greater. In 25 patients, acute pressure changes were induced by volume expansion, pachng, or drug infusion ( h o p terenol or phenylephrine). Increases in left atrial pressure hquently failed to be reflected in right atrial pressure changes. CenW venous pressare may be misleading as a guide to left atrial pressure.

SHORT TERM CLINICAL OUTCOME OF PERCUTANEOUS BALLOON MITRAL VALVULOPLASTY VERSUS SURGICAL CLOSED MITRAL COMMISSUROTOMY
BaIlon mitral valvuloplasty and closed mitral valve conunissurotomy were performed in 450 and 127 patients, respectively. Also, 254 patients with severe mitral stenosis were enrolled in a prospective randomized trial comparing the two procedures. Immediately after balloon mitral valvuloplasty, mean left atrial pressure was 11.38±3.54 mmHg, transmitral valve gradient was 1.8±2 mmHg, and mit...
متن کاملRight and left heart pressures in acute myocardial infarction.
AUTHORS' SYNOPSIS In dogs the effects of acute myocardial infarction (AMI) on right and left heart pressure relationships were studied. Right atrial pressure was a poor predictor of left heart pressures. Pulmonary artery end-diastolic, left atrial, and left ventricular end-diastolic pressures tended to equalize in normals (except with tachycardia) and after AMI. In a previously scarred left ven...
متن کاملAssociation between Body Mass Index and Mitral Valve Prolapse
Introduction: Body mass index (BMI) can affect cardiac morphology; however, the relationship between BMI and valvular heart diseases has not been thoroughly evaluated. This study aimed to determine the relationship between BMI and mitral valve prolapse (MVP) as one of the most common valve diseases worldwide. It can help us to better understand pathophysiology of this common disease. Materials ...
متن کاملHigh-risk Coronary Artery Bypass Grafting and Mitral Valve Replacement in a HIV Positive Patient
Certain subsets of high-risk mitral valve patients are not suitable candidates for transcatheter therapies. The objective of this report is to present a young patient with combined mitral valve and coronary artery disease to illustrate these challenges. In this report, we present a 47-year-old man with longstanding HIV infection who was referred with severe mitral regurgitation (MR) and profound...
متن کاملEffects of inhaled nitric oxide on pulmonary hypertension in patients after mitral valve surgery
Objective: Cardiac surgery in patients with pulmonary hypertension may present severe postoperative complications. The study consists of verifying the effects of using inhaled nitric oxide (iNO) in patients during the postoperative period of mitral valve surgery. Methods: The effects of iNO were measured mainly by verifying changes in pulmonary artery pressure (PAP). Other measures performed in...
